AI Receptionist for Allied Health Clinics: What It Does
An AI receptionist for allied health clinics is software that answers patient phone calls for a physiotherapy, chiropractic, podiatry, psychology or similar practice, checks the clinic's live calendar, and books, reschedules or cancels appointments without a staff member picking up. It also answers common questions from the clinic's own information and hands anything clinical or urgent to a person. For a clinic on Cliniko, it's the difference between a patient who gives up after the fifth ring and one who's booked for Thursday at 2pm.
Key facts
- Medow Health AI said in September 2026 that up to half of patient calls to specialist medical practices can go unanswered when reception is stretched (Startup Daily).
- 51% of online health bookings on HotDoc's platform were made between 5pm and 9am, outside most front-desk hours (HotDoc).
- Cliniko's connected-apps directory lists 38 AI apps, about 13 of which are pitched as AI receptionists or phone answering.
- Health information is "sensitive information" under the Privacy Act, and allied health providers are covered regardless of turnover, so data location is a fair first question for any vendor.
Why missed calls are getting attention now
On 16 September 2026, Melbourne-based Medow Health AI announced a A$3.5 million seed extension, its third raise, bringing its total to A$8.5 million since January 2025. The money is going into expanding from AI scribing into an "AI Front Desk" for specialist clinics. Cofounder Joel Freiberg's stated reason, reported by Startup Daily, was that up to half of patient calls to specialist clinics can go unanswered when reception is stretched.
Medow sells to specialist medical practices, not allied health, and it isn't a Cliniko product. But the problem underneath it doesn't stop at the specialist's door. A five-practitioner physio clinic's front desk is interrupted by the same things: someone at the counter, a call on hold, a practitioner with a question between sessions. And allied health clinics often run leaner reception than medical practices, with one or two people covering phones, check-ins, payments and clinician support at once.
What an AI receptionist for allied health clinics does
A genuine AI receptionist does four things a voicemail, answering service or website chatbot doesn't.
It reads the live calendar. It checks real availability in Cliniko, including appointment types, durations and each practitioner's hours, rather than taking a message for someone to reconcile later.
It follows your booking rules. "Initial assessments only before 3pm", "leave 10 minutes after two back-to-back sessions", "new chiro patients only with Dr Sarah". A receptionist that ignores those rules creates the double-bookings your front desk then has to unpick.
It keeps the conversation going when the first answer is no. If the patient's preferred slot is gone, it offers the next one, or another practitioner, on the same call instead of ending with "someone will call you back".
It books, reschedules or cancels in the same calendar your staff use, and texts the patient a confirmation, so nothing is re-keyed next morning.
The retry step separates a booking AI receptionist from a message-taking one.
It also answers routine questions from the clinic's own content: parking, what to bring, fees and gaps, and how GP Chronic Condition Management Plan referrals work at your practice (since 1 July 2025 these have replaced GP Management Plans and Team Care Arrangements, and patients can still claim up to five Medicare allied health services a year, per the Australian Physiotherapy Association).
What it shouldn't do
An AI receptionist is a receptionist. It shouldn't assess symptoms, triage clinically or give treatment advice. The clinic decides what counts as urgent and who gets the hand-over. For emergencies the caller is told to call 000. Metilda passes anything urgent or outside its remit to staff or to the emergency contacts the clinic nominates.
It also doesn't replace a practice manager. Most clinics use one to cover after-hours calls, overflow while staff are busy, and gaps from leave or turnover. We look at that last point in the real cost of front-desk staff turnover.
What to check before buying one
- Is it built for your practice software? Cliniko-specific tools (Lyngo AI, Aeva AI, Metilda and others) read and write appointments directly. General voice platforms often need custom work per clinic. Our five tests for Cliniko integration claims go deeper.
- What happens when the requested slot is taken? Ask for a live demo against a full calendar.
- Can you read the transcript of any call? Not a summary score. See why transcripts matter.
- Where is patient data processed and stored? Get it in writing.
- Is pricing published? Many vendors keep it behind a sales call. Grounded Scribe is an exception, publishing plans from $19 a month.
- Does it sound like a clinic or a call centre? This one doesn't fit on a comparison grid, but it's what patients notice.
Where Metilda fits
Metilda is an AI receptionist built for allied health clinics on Cliniko. We describe it as the most humane AI receptionist for allied health. That's a narrow claim on purpose: not the longest feature list, but a call a patient doesn't feel talked down on. The checkable parts are the ones above: live Cliniko booking with your rules, retries instead of dead ends, hand-over rules you write, and a portal where you can read every call.

Patient data
Health information is "sensitive information" under the Privacy Act (OAIC Guide to health privacy), and the OAIC confirms allied health professionals are covered even as small businesses. Metilda's servers and language models are hosted in Australia, so patient details aren't sent offshore for processing. Calls, transcripts and booking records are encrypted in transit and at rest, and each clinic's records are isolated. We don't claim certifications we don't hold.
FAQ
What is an AI receptionist for allied health clinics?
An AI receptionist for allied health clinics is software that answers patient phone calls for a physiotherapy, chiropractic, podiatry, psychology or similar practice, checks the live appointment calendar, and books, reschedules or cancels appointments while following the clinic's booking rules. It answers routine questions from the clinic's own information and hands clinical or urgent calls to staff.
How many patient calls go unanswered at clinics?
There's no verified figure for allied health specifically. Medow Health AI told Startup Daily in September 2026 that up to half of patient calls to specialist medical practices can go unanswered during busy periods. Allied health clinics, which often run leaner reception, have no obvious reason to do better.
Does an AI receptionist replace front-desk staff?
Usually not. Most clinics use one for after-hours calls, overflow while staff are busy, and cover during leave or turnover, while the practice manager handles in-person tasks and anything needing judgment.
Does Metilda only work with Cliniko?
Yes. Metilda is built specifically for clinics on Cliniko, which is why it can read and write the clinic's live appointment calendar directly.
Is patient data safe with an AI phone system?
It depends on the vendor, so ask where data is processed and stored and get it in writing. Metilda's servers and language models are hosted in Australia, data is encrypted in transit and at rest, and each clinic's records are isolated from other clinics'.
